When the people we support are ready to move back into community living, we understand the importance of careful planning and inter-agency coordination. Working in collaboration with commissioners, we support a seamless continuity of care.

When individuals make the move on from hospital we are able to offer step-down residential services through Ocean Community Services, as well as supported living services as part of our pathway, Our small homes offer support to both men and women with mental health needs, learning disabilities and acquired brain injury, including those with forensic history and are located across South Wales and South West England.

Located in areas which encourage positive integration with local communities, our family-style homes cater for individuals at every stage of their journey; offering them a secure base, supportive relationships, social inclusion, empowerment and the further development of coping strategies and independent living skills.
